Annual Lifeline Recertification

To remain eligible to participate in the Lifeline program, each calendar year, all subscribers must verify that they are still eligible for the program. Subscribers who fail to recertify their eligibility will be de-enrolled from the program and lose their Lifeline benefit.
During the re-certification, subscribers are asked to confirm that their information is current and then read a number of statements, checking them off to indicate that they understand the rules of Lifeline. It is similar to the form that was filled out during the initial enrollment in the program. Once the form is signed and submitted, the certification is complete.
Some states, including Pennsylvania, require documentation of eligibility be provided. To complete re-certification, a picture of your qualifying program benefit card or letter of participation must be submitted in addition to the re-certification form.

How to Re-Certify

There are multiple ways that you can re-certify.

Note: Pennsylvania customers will need to send proof of eligiblity in addition to completing the re-certification form.

Who needs to re-certify?

All subscribers who originally enrolled in the Lifeline program in a year prior to the current year.

When can re-certification be done?

Re-certification can be done at any time during the year. It must be completed before the end of the calendar year.

What happens if I don't complete the re-certification by the end of the year?

Those subscribers that don't complete the re-certification during the year will be de-enrolled from Lifeline and no longer received lifeline supported service.

I'm in Pennsylvania and I submitted my re-certification on-line. What else do I need to do?

Pennsylvania requires Lifeline participants to submit proof of continued eligibility each year. To complete your certification you will need to submit proof of your qualifying program. For example: A picture of your SNAP/Foodstamps card, or a picture of your medicaid card. You can subit the proof by emailing it, faxing it, or mailing it to us (see below).

Pennsylvania - Proof of Qualification

Please provide us with a picture or copy of one of the following that pertains to your qualification.

Program Based Eligibility

  • Benefit Card (SNAP, Medicaid, etc)
  • Notice Letter of Participation
  • Benefit Check Stub from SSA
  • Public Housing Lease Agreement
  • Utility bill showing assistance credit
  • Statement of beneifts

Income Based Eligibility

  • Prios Year's state, federal, of Tribal tax return
  • Current Income Statement
  • Paycheck stubs showing at least 3 consecutive months income
  • Veterans Administration statement of benefits
  • Social Security statement of benefits
  • Pension / Retirement statement of benefits
  • Unemployment / Workmans comp statement of benefits
  • Federal or Tribal notice letter of participation in General Assistance
  • Child support award, divorce decree, or other official document
    showing at least 3 consecutive months of income
